Accessible Furniture Request Form

This form is only for those students who have been approved for Accessible Furniture on the Disability Support Accommodations Agreement.  In addition, you only need to request furniture that is not standard in most classrooms.  In the form below, you will have the options to select commonly requested furniture.  Please make sure that you only select furniture which you have a specific need for based on your disability.

This form needs to be submitted each semester after enrolling in classes.  Please submit this form at least 10 business days before your classes begin to ensure that the furniture can be placed in time.
