Academic Suspension Appeal Form

You may appeal your Academic Suspension for the following reasons, however the acceptance of your appeal is not automatically guaranteed:

Extenuating Circumstances

  • Serious Illness
  • Unexpected Economic Hardship
  • Death in family

Error

You will be asked to submit third party documentation (obituary, medical documentation, etc.). Make sure that any submissions contain your name, student ID number and state that they are documentation for your Academic Suspension Appeal. Documentation can be mailed, faxed or hand delivered to:

Please note that students returning from Academic Suspension will be required to meet with a counselor prior to being able to register for classes.
