Procedure for turning in Disability Test Request Forms

Please be sure to show your instructor a copy of your Accommodation Agreement during the first week of class, you will not be allowed to test until this is done. Students are responsible for ensuring that this form is completely filled out and returned to the Disability Testing office at least 3 weekdays prior to scheduled testing. Forms for Final Exams must be turned in a minimum of 7 weekdays prior to testing. (Holidays, weekends & days the school is closed do not count as weekdays). You are allowed to turn in 1 late form per semester, this is considered an “OOPS”. After that if another late form is turned in, you will need to take the test in the classroom without accommodations. You cannot use your “OOPS” for final exams.

You are expected to take your test at the SAME TIME you would in the classroom unless you have a scheduling conflict or the instructor’s permission. GRCC’s Disability Support Services Office maintains a high standard of academic and personal honesty when providing testing accommodations. If a student is caught cheating—the test, notes, and any unauthorized materials will be given to the instructor and the Student Code of Conduct office will be notified. Students: Please remember that it is YOUR responsibility to remind your instructor of your test date and time. This will ensure that your test is delivered on time. Students please be advised you will be monitored &/or recorded while in testing rooms.

 

If you turn your form in: The earliest you can test is:
Monday by 6:30 pm Thursday
Tuesday by 6:30 pm Friday
Wednesday by 6:30 pm Monday
Thursday by 6:30 pm Tuesday
Friday by 4:00 pm Wednesday
